Housecall Providers in Portland, Oregon is seeking a Licensed Practical Nurse to support hospice care under a registered nurse. This PRN role requires 40–50 hours of availability per month in the Portland area and adherence to the patient’s plan of care.
Responsibilities include direct patient care, administering medications, collaborating with the interdisciplinary team, documenting in EMR, and providing patient and caregiver education.
The Licensed Practical Nurse assists in planning, coordination and care of hospice patients in accordance with the patient’s plan of care. The position provides direct patient care under the supervision of a registered nurse. Responsibilities include following the plan of care, providing treatments, and working collaboratively with the members of the interdisciplinary team to help achieve positive care outcomes.
NOTE: This hospice LPN position is PRN. Must have at least 40‑50 hours of availability per month in the Portland area.
Estimated Hiring Range: $33.98 - $41.53
Bonus Target: Bonus - SIP Target, 5% Annual

Schedule: This position may occasionally require more than 8 hours per day and may be assigned on-call or after-hours duties.
