Job summary

Qualifications

• GED/High School diploma or higher required.
• Associate degree or higher with relevant licenses preferred.
• Master’s degree or higher with appropriate licensure preferred.

Responsibilities

• Complete patient databases, treatment plans, progress notes, discharge summaries per standards.
• Provide orientation on program objectives, phase requirements, rules, sanctions, hours of service, patient rights.
• Counsel patients in crisis and arrange referrals to community resources.
• Maintain clinical documentation and patient charts per policies.
• Submit monthly/quarterly statistics to supervisor.
• Attend supervision and trainings as required.
