Qualifications

• Counselor I–III requirements include OH licensure or better.
• GED/HS diploma or higher required.
• OH licensure or certification at licensure levels is required.

Responsibilities

• Complete patient databases, treatment plans, progress notes, discharge summaries, and other documentation.
• Orient patients to program objectives, phase requirements, rules, and hours of services.
• Assess and place patients according to treatment phases per standards.
• Update clinical documentation and charts per policies.
• Counsel patients in crisis and guide coping resources.
• CounselPatients who fail to comply and explain consequences.
• Arrange referrals to community resources when appropriate.
• Participate in case conferences and reviews; learn confidentiality and methadone regs.
• Submit internal/external correspondence for supervisor review.
• Complete intakes and facilitate group counseling sessions.
• Collect urine drug screens following chain of custody.
• Obtain necessary documentation to ensure policy compliance.
• Submit monthly/quarterly reports to supervisor.
• Attend scheduled supervision and team meetings; complete trainings.
• Other duties as assigned.
