Qualifications

• Must be Ohio resident.
• Active Behavioral Health license in the state Ohio (e.g., LMFT, LSW, LPC, LISW).
• 3+ years of direct clinical practice post‑Master’s degree.
• 2+ years in children’s mental health or related community services.
• 2+ years in case management, family therapy, or related fields.
• Willing to travel up to 10% for trainings/meetings in OH.
• Mon-Fri, 8am-5pm.
• 1+ year of experience with MS Office.

Responsabilités

• Conduct comprehensive assessments of needs and eligibility using clinical tools and data.
• Apply clinical judgment to care planning and resource coordination.
• Perform crisis intervention and follow-up for members in crisis.
• Develop and monitor plans of care with care teams and providers.
• Collaborate with Medical Directors to overcome barriers and optimize care.
• Escalate quality of care issues as needed.
• Engage members to promote health and lifestyle changes.
