Qualifications

• Must be Ohio Resident.
• Active Behavioral Health license in the state Ohio (e.g. LMFT, LSW, LPC, LISW).
• 3+ years of direct clinical practice experience post-Master's degree, e.g., hospital setting or ambulatory care.

Responsibilities

• Conduct comprehensive assessments of referred members to determine needs, eligibility and care plan.
• Coordinate care planning and direct provider collaboration to optimize outcomes.
• Perform crisis intervention and follow-up to ensure appropriate treatment and services.
• Apply clinical guidelines and standardized care plans to ensure appropriate administration of benefits and services.
