• CCC: Assists intake and OASIS team with compliant F2F documentation from SOCs. Obtains clinical report from RN on all SOC, ROC and RC OASIS visits and monitors/facilitates RN communication regarding plan of care.

Obtains clinical report from RN on all SOC, ROC and RC OASIS visits and monitors/facilitates effective RN communication regarding plan of care.
