Job summary

Be Healthy at Home is seeking a healthcare coordinator to support patients under the direction of the RN. You will triage patient phone calls, coordinate care between practitioners, patients, and staff, and ensure visits are scheduled when needed.

You will obtain and transcribe orders, provide education to patients and caregivers, and support discharge preparation. In this role you will participate in Intake on-call rotation, promote infection control practices, and contribute to QAPI

Job Summary:
Under the direction of the RN, triages patient phone calls, collaborating with Scheduler to ensure PRN visits
are scheduled when necessary.
Coordinates care between practitioners, patients, and field/office staff. Provides effective communication to
patients, family members, Agency team members, and other healthcare professionals.
Obtains practitioner verbal orders when necessary and transcribes orders into EMR. Receives and
transcribes external orders into EMR.
Under the direction of the RN, provides appropriate nursing interventions, provides patient and caregiver
education, and prepares patient and caregiver for discharge from services. Implements infection control
measures to protect both the staff and the patient.
Performs clinical review of incoming referrals, coordinating with Scheduler on staffing capacity and Branch
Director on potential declined referrals.
Communicates with Account Executives, referral partners, patients, and certifying practitioners to obtain all
necessary referral components.
Demonstrates commitment and professional growth by participating in Agency-sponsored in-services and
maintaining or improving competency. Participates in the Agency's QAPI program.
Promotes the Agency's philosophy and administrative policies.
Performs patient visits as needed. Participates in Intake on-call rotation.
