Job summary

The South Dakota Department of Corrections in Sioux Falls seeks a Corrections Case Manager to conduct intake assessments, compile histories, and develop initial case management plans aligned with offender programming and safety.

You will retrieve data from internal systems, coordinate access to work, education, treatment, and housing resources, and monitor progress with a multidisciplinary team to support successful re-entry.

Qualifications

• Three (3) years of experience working directly with a hard to serve high needs client population in a correctional or human services environment providing case management.
• Experience in offender management, classification, and program referrals is preferred.
• Knowledge of community resources and ability to coordinate services with multiple agencies.

Responsibilities

• conduct an initial assessment and admission data summary that includes criminal history, social history, medical, dental, mental health needs, occupational experience and interests, vocational programming, education, and previous information reported by the SD DOC or other criminal justice organizations
• retrieve and enter information by accessing SD DOC information systems and utilizing internal and external data sources
• analyze retrieved data and investigate for linked information
• meet with individual offenders to identify and collect meaningful information to develop an initial case management plan
• coordinate offender access to work, education, treatment and recreational programs, determine earned time grants, develop initial parole board risk assessments and actions, and design parole plans
• monitor, evaluate, and record progress; complete assessments and updates at regular intervals in accordance with SD DOC policy and case management procedures
