Job summary

Boost Home Healthcare - Northern Colorado is seeking an Occupational Therapist to provide in-home therapy under physician orders, within the Director/Clinical Supervisor's direction. The role supports patients to remain independent at home.

Responsibilities include initial assessment, developing and reassessing a 30-day plan of care, collaboration with other home care staff, and coordinating with physicians to modify treatment as needed. Part-time or PRN schedule; travel required.

Qualifications

• Graduate of an OT curriculum accredited by the AMA/AOTA.
• Eligible for NBCOT Certification.
• Licensed to practice occupational therapy in the practicing state.
• Minimum 1 year in acute care; 2 years Home Health preferred.

Responsibilities

• Improve or minimize residual physical disabilities of the patient.
• Return the individual to optimum and productive living within the patient’s capability.
• Periodically participate with all other home care personnel in patient’s care planning.
• Provide prescribed occupational therapy and skilled procedures.
• Initial assessment, plan of care maintenance and 30-day reassessments.
• Consult with physicians regarding changes in treatment.

An Occupational Therapist (OT) administers occupational therapy to patients on an intermittent basis in their place of residence. This is performed in accordance with physician orders and plan of care under the direction and supervision of the Director of Clinical Services/Clinical Supervisor/Manager.

What you’ll be doing:

• Responsible for initial assessment, plan of care maintenance program development and modifications and reassessments every 30 days.
• Consult with physicians regarding change in treatment.

What we’re looking for:

• Graduate of an Occupational Therapy curriculum accredited by the Committee on Allied Health Education and Accreditation of the American Medical Association and the American Occupational Therapy Association (AOTA) and /or certified by successfully completing the National Certification examination, -or-
• Eligible for the National Registration Examination of the American Occupational Therapy Association
• Currently licensed by the Board of Occupational and Physical Therapy Examiners in the state in which practicing, unless licensure does not apply
• Minimum of one year in an acute care setting.
• Two (2) years’ experience in Home Health preferred.
• Access to reliable transportation
