Job summary

Evolve Therapy Services, LLC is seeking a Certified Occupational Therapy Assistant for an in-house Skilled Nursing Facility in Olivia, MN. You will plan, organize, develop, and direct Occupational Therapy Services to ensure high-quality resident care in compliance with standards.

Responsibilities include screenings, treatment plans, communicating findings to residents and families, scheduling, documentation, and CMS-compliant billing.

Qualifications

• Associate's degree in Occupational Therapy required.
• MUST have active OTA licensure in state(s) of practice.
• Ongoing continuing education and professional development to maintain licensure and certification.

Responsibilities

• Conduct regular screening of residents to determine best intervention or treatment plan.
• Perform treatment plans aligned with goals and managed by supervising therapist.
• Communicate evaluation findings, treatment plans, and recommendations to residents, families, and team members.
• Establish resident therapy schedules in collaboration with team members.
• Document and bill occupational therapy services in accordance with policy.
