Responsibilities

• Follows the therapist's plan of care and adjusts treatment based on patient response with communication to evaluating therapist.
• Implements patient-centered, occupation-based interventions and adapts activities to meet functional and emotional needs.
• Collaborates with patient, family, and health care team to support rehabilitation goals.
• Educates patients, families, and caregivers using effective teaching methods.
• Documents progress clearly and accurately for payers and care team.
• Communicates changes in patient status and discharge needs to the care team.
