Job summary

Huebner Creek Health in San Antonio, TX is seeking a Per Diem Occupational Therapy Assistant (OTA) to join our in-house rehab team. This PRN position focuses on delivering patient-centered therapy to help residents improve independence and quality of life.

The OTA will work under an OT, perform treatments, document progress, and communicate with families and caregivers. Texas license and COTA credentials are required; SNF experience is preferred, with familiarity using Net Health EDS.

Qualifications

• Active and valid Texas OTA license.
• COTA credential required.
• In good standing with licensing boards.
• SNF or long-term care experience preferred.
• Knowledge of Net Health EDS system preferred.
• Strong communication and teamwork skills.
• Ability to manage patients with varying needs.

Responsibilities

• Plan and administer occupational therapy treatments to residents and patients with temporary or long-term disabilities to relieve pain, restore or improve function, and promote healing.
• Provide skilled occupational therapy services and interventions in accordance with physician orders under the supervision of an Occupational Therapist.
• Respond to requests for service by relaying information and referrals to the Occupational Therapist.
• Ensure all treatments are delivered in accordance with the established plan of care.
• Provide clinical support and instruction to patients, families, and caregivers.
• Monitor patient response to treatment interventions and report changes as appropriate.
• Complete required documentation thoroughly and timely in accordance with company policy and state/federal regulations.
• Attend required meetings as designated by the Director of Rehab.

In this role, you will provide person‑centered care to help residents achieve positive outcomes and improve their independence and quality of life.
