Job summary

Evolve Therapy Services, LLC is hiring a Certified Occupational Therapy Assistant for an in-house Skilled Nursing Facility in Olivia, MN. The role focuses on planning, directing and delivering occupational therapy services to residents in accordance with federal, state, and local standards.

Full-time opportunities are available with a $5,000 sign-on bonus and tuition reimbursement. Part-time or PRN positions may also be offered, with benefits commensurate with status.

Qualifications

• Active licensure required for OT practice in state(s) of residence.
• Associate's degree in Occupational Therapy required.
• Ongoing continuing education and professional development to maintain licensure and certification.

Responsibilities

• Conduct regular resident screenings to determine interventions according to scope of practice.
• Develop and implement treatment plans aligned with short- and long-term goals.
• Communicate evaluation findings and recommendations to residents, families, and interdisciplinary team.
• Coordinate therapy schedules with team members to ensure coverage.
• Document therapy sessions and ensure proper billing and compliance with CMS standards.

Job Position:

Certified Occupational Therapy Assistant for an In-house Skilled Nursing Facility

Job Type:

Full Time ($5,000 sign on + Tuition Reimbursement!), Part Time or PRN positions available

Job Purpose:

To plan, organize, develop, and direct Occupational Therapy Services in facility in accordance with current applicable federal, state, and local standards, guidelines, and regulations, and as may be directed by Administrator, to ensure that highest degree of quality resident care can be maintained at all times.

Job Functions:

• Conduct regular screening of residents to figure out the best intervention or treatment for each individual patient based off scope of practice


• Perform treatment plan to residents; follow short- and long-term goals and methods to achieve goals based off of the established plan of care by supervising therapist


• Interpret and communicate evaluation findings, treatment plans, and recommendations to residents, families/caregivers, and interdisciplinary team members


• Establish individual resident therapy schedules in cooperation with other team members


• Conduct therapy according to established treatment plan and ensure proper documentation and billing of occupational therapy services


• Perform all other duties as assigned and as required to effectively treat patients


• Comply with all CMS documentation standards of practice daily with point of care documentations as indicated

Education & Qualifications:

• Associate's degree in Occupational Therapy


• MUST have active OTA licensure in state(s) of practice


• Ongoing continuing education and professional development to maintain current licensure and certification
