Job summary

Evolve Therapy Services, LLC in Franklin, MN is hiring a Certified Occupational Therapy Assistant to provide in-house OT services in a Skilled Nursing Facility. Full-time, part-time, or PRN positions are available with a $5,000 sign-on bonus plus tuition reimbursement.

You will screen residents, develop and follow treatment plans, communicate with families and the interdisciplinary team, schedule therapy, document and bill services, and ensure CMS standards are met to maintain high-quality

Qualifications

• Must hold active OTA licensure in state(s) of practice.
• Associate's degree in Occupational Therapy.
• Ongoing continuing education to maintain licensure and certification.

Responsibilities

• Screen residents to determine best intervention for each patient.
• Perform treatment plans and progress toward goals per care plan.
• Communicate evaluation findings and recommendations to residents, families, and the interdisciplinary team.
• Establish individual resident therapy schedules with the team.
• Conduct therapy per treatment plan and document/bill occupational therapy services.
• Perform other duties as assigned to effectively treat patients.
• Comply with CMS documentation standards in daily documentation.

Job Position:

Certified Occupational Therapy Assistant for an In-house Skilled Nursing Facility

Job Type:

Full Time ($5,000 sign on + Tuition Reimbursement!), Part Time or PRN positions available

Job Purpose:

To plan, organize, develop, and direct Occupational Therapy Services in facility in accordance with current applicable federal, state, and local standards, guidelines, and regulations, and as may be directed by Administrator, to ensure that highest degree of quality resident care can be maintained at all times.

Job Functions:

• Conduct regular screening of residents to figure out the best intervention or treatment for each individual patient based off scope of practice
• Perform treatment plan to residents; follow short- and long-term goals and methods to achieve goals based off of the established plan of care by supervising therapist
• Interpret and communicate evaluation findings, treatment plans, and recommendations to residents, families/caregivers, and interdisciplinary team members
• Establish individual resident therapy schedules in cooperation with other team members
• Conduct therapy according to established treatment plan and ensure proper documentation and billing of occupational therapy services
• Perform all other duties as assigned and as required to effectively treat patients
• Comply with all CMS documentation standards of practice daily with point of care documentations as indicated

Education & Qualifications:

• Associate's degree in Occupational Therapy
• MUST have active OTA licensure in state(s) of practice
• Ongoing continuing education and professional development to maintain current licensure and certification
