Evolve Therapy Services is seeking a Certified Occupational Therapy Assistant to provide in-house therapy services at a Skilled Nursing Facility in Beloit, WI. This role requires an Associate's degree in occupational therapy and active OTA licensure in the state.

The position offers Full Time, Part Time or PRN opportunities with competitive pay and benefits. You will conduct screenings, implement treatment plans, and document per CMS standards while collaborating with an interdisciplinary team

Qualifications

• Associate's degree in occupational therapy required.
• Active OTA licensure in state(s) of practice required.
• Ongoing continuing education to maintain licensure and certification.

Responsabilités

• Conduct regular screenings of residents to determine appropriate interventions.
• Implement treatment plans and follow established goals.
• Communicate evaluation findings and recommendations to residents and interdisciplinary team.
• Schedule resident therapy in collaboration with team members.
• Perform therapy per treatment plan and ensure proper documentation and billing.
• Comply with CMS documentation standards daily with point-of-care documentation.
• Perform other duties as assigned to effectively treat patients.

Job Position:

Certified Occupational Therapy Assistant for an In-house Skilled Nursing Facility

Job Type:

Full Time, Part Time or PRN positions available

Job Purpose:

To plan, organize, develop, and direct Occupational Therapy Services in facility in accordance with current applicable federal, state, and local standards, guidelines, and regulations, and as may be directed by Administrator, to ensure that highest degree of quality resident care can be maintained at all times.

Job Functions:

• Conduct regular screening of residents to figure out the best intervention or treatment for each individual patient based off scope of practice
• Perform treatment plan to residents; follow short and long term goals and methods to achieve goals based off of the established plan of care by supervising therapist
• Interpret and communicate evaluation findings, treatment plans, and recommendations to residents, families/caregivers, and interdisciplinary team members
• Establish individual resident therapy schedules in cooperation with other team members
• Conduct therapy according to established treatment plan and ensure proper documentation and billing of occupational therapy services
• Perform all other duties as assigned and as required to effectively treat patients
• Comply with all CMS documentation standards of practice daily with point of care documentations as indicated

Education & Qualifications:

• Associate's degree in occupational therapy
• MUST have active OTA licensure in state(s) of practice
• Ongoing continuing education and professional development to maintain current licensure and certification
