Evolve Therapy Services, LLC is hiring an Certified Occupational Therapy Assistant for an in-house Skilled Nursing Facility in Greendale, WI. This full-/part-time/PRN position offers the chance to plan, direct, and deliver OT services that enhance resident care within a CMS-compliant setting.

Qualifications

• Associate's degree in Occupational Therapy.
• Must have active OTA licensure in state(s) of practice.
• Ongoing continuing education and professional development to maintain licensure and certification.

Responsibilities

• Conduct regular screening of residents to determine the best intervention for each patient.
• Perform treatment plans and follow short- and long-term goals per plan of care by supervising therapist.
• Interpret and communicate evaluation findings, treatment plans, and recommendations to residents, families, and interdisciplinary team members.
• Establish individual resident therapy schedules in cooperation with team members.
• Conduct therapy according to the treatment plan and ensure proper documentation and billing of occupational therapy services.
• Perform all other duties as assigned to effectively treat patients.
• Comply with CMS documentation standards of practice daily with point-of-care documentation.

Job Purpose

To plan, organize, develop, and direct Occupational Therapy Services in facility in accordance with current applicable federal, state, and local standards, guidelines, and regulations, and as may be directed by Administrator, to ensure that highest degree of quality resident care can be maintained at all times.

Job Functions
