Job summary

Evolve Therapy Services, LLC is seeking a Certified Occupational Therapy Assistant for an in-house Skilled Nursing Facility in Madison, WI. This PRN role focuses on planning, directing, and delivering occupational therapy services to residents in accordance with all standards and regulations.

The OTA will conduct screenings, implement treatment plans, maintain documentation, and collaborate with the interdisciplinary team to ensure high-quality resident care.

Qualifications

• Associate degree in Occupational Therapy is required.
• Active OTA licensure in state(s) of practice is required.
• Ongoing continuing education and professional development to maintain licensure and certification.

Responsibilities

• Screen residents to determine best intervention or treatment.
• Develop and implement treatment plans and goals.
• Communicate evaluation findings to residents, families, and the care team.
• Schedule resident therapy in coordination with team members.
• Document and bill occupational therapy services.
• Adhere to CMS documentation standards of practice daily.

Job Position

Certified Occupational Therapy Assistant for an In-house Skilled Nursing Facility

Job Purpose

To plan, organize, develop, and direct Occupational Therapy Services in facility in accordance with current applicable federal, state, and local standards, guidelines, and regulations, and as may be directed by Administrator, to ensure that highest degree of quality resident care can be maintained at all times.

Job Functions

• Conduct regular screening of residents to figure out the best intervention or treatment for each individual patient based off scope of practice
• Perform treatment plan to residents; follow short- and long-term goals and methods to achieve goals based off of the established plan of care by supervising therapist
• Interpret and communicate evaluation findings, treatment plans, and recommendations to residents, families/caregivers, and interdisciplinary team members
• Establish individual resident therapy schedules in cooperation with other team members
• Conduct therapy according to established treatment plan and ensure proper documentation and billing of occupational therapy services
• Perform all other duties as assigned and as required to effectively treat patients
• Comply with all CMS documentation standards of practice daily with point of care documentations as indicated

Education & Qualifications

• Associate's degree in Occupational Therapy
• MUST have active OTA licensure in state(s) of practice
• Ongoing continuing education and professional development to maintain current licensure and certification
