Job summary

Evolve Therapy Services is hiring a Certified Occupational Therapy Assistant for an in-house Skilled Nursing Facility in Fort Atkinson, WI. This PRN role supports the therapy team with resident-focused occupational therapy services and ongoing documentation in a facility setting.

Responsibilities include conducting screenings, executing treatment plans, and collaborating with a multidisciplinary team to ensure quality resident care while maintaining CMS documentation standards.

Qualifications

• Associate's degree in Occupational Therapy.
• MUST have active OTA licensure in state(s) of practice.
• Ongoing continuing education and professional development to maintain current licensure and certification.

Responsibilities

• Conduct regular resident screenings to identify the best intervention for each patient.
• Implement treatment plans; follow short- and long-term goals per the plan of care.
• Interpret and communicate evaluation findings to residents, families/caregivers, and the interdisciplinary team.
• Establish individual resident therapy schedules in cooperation with other team members.
• Conduct therapy according to the established treatment plan and ensure proper documentation and billing of occupational therapy services.
• Perform all other duties as assigned and required to effectively treat patients.
• Comply with CMS documentation standards of practice daily with point-of-care documentation as indicated.

Job Position: Certified Occupational Therapy Assistant for an In-house Skilled Nursing Facility

Job Type: PRN

Job Purpose: To plan, organize, develop, and direct Occupational Therapy Services in facility in accordance with current applicable federal, state, and local standards, guidelines, and regulations, and as may be directed by Administrator, to ensure that highest degree of quality resident care can be maintained at all times.

Job Functions

• Conduct regular screening of residents to figure out the best intervention or treatment for each individual patient based off scope of practice
• Perform treatment plan to residents; follow short- and long-term goals and methods to achieve goals based off of the established plan of care by supervising therapist
• Interpret and communicate evaluation findings, treatment plans, and recommendations to residents, families/caregivers, and interdisciplinary team members
