Job summary

Evolve Therapy Services is seeking a Certified Occupational Therapy Assistant for our partner in Milwaukee, WI, to plan, organize, and direct occupational therapy services in a skilled nursing facility.

You will screen residents, develop treatment plans with supervising therapist, ensure documentation and billing, and coordinate with the interdisciplinary team. Requires Associate’s degree and active OTA licensure; ongoing CEU encouraged.

Qualifications

• Associate's degree in Occupational Therapy.
• Must have active OTA licensure in state(s) of practice.
• Ongoing continuing education and professional development to maintain licensure.

Responsibilities

• Conduct regular screening of residents to determine best intervention or treatment.
• Perform treatment plans with residents; follow short- and long-term goals as established by supervising therapist.
• Interpret and communicate evaluation findings, treatment plans, and recommendations to residents, families, and the interdisciplinary team.
• Establish individual resident therapy schedules with team members.
• Conduct therapy per treatment plan and ensure documentation and billing of occupational therapy services.
• Perform all other duties as assigned to effectively treat patients.
• Comply with CMS documentation standards of practice daily with point-of-care documentation.

Education

Associate's degree in Occupational Therapy

Active OTA licensure in state(s) of practice

Job Position: Certified Occupational Therapy Assistant for an In-house Skilled Nursing Facility

Job Type: Full Time

Job Purpose: To plan, organize, develop, and direct Occupational Therapy Services in facility in accordance with current applicable federal, state, and local standards, guidelines, and regulations, and as may be directed by Administrator, to ensure that highest degree of quality resident care can be maintained at all times.

Job Functions:

• Conduct regular screening of residents to figure out the best intervention or treatment for each individual patient based off scope of practice

• Perform treatment plan to residents; follow short- and long-term goals and methods to achieve goals based off of the established plan of care by supervising therapist

• Interpret and communicate evaluation findings, treatment plans, and recommendations to residents, families/caregivers, and interdisciplinary team members

• Establish individual resident therapy schedules in cooperation with other team members

• Conduct therapy according to established treatment plan and ensure proper documentation and billing of occupational therapy services

• Perform all other duties as assigned and as required to effectively treat patients

• Comply with all CMS documentation standards of practice daily with point of care documentations as indicated

Education & Qualifications:

• Associate's degree in Occupational Therapy

• MUST have active OTA licensure in state(s) of practice

• Ongoing continuing education and professional development to maintain current licensure and certification
