Job summary

Evolve Therapy Services, LLC is seeking a Certified Occupational Therapy Assistant for an in-house Skilled Nursing Facility in Manitowoc, WI. Part-time or PRN positions available.

You will plan, organize, develop, and direct OT services, conduct resident screenings, implement treatment plans, and collaborate with the interdisciplinary team to ensure quality care.

Candidates must have an Associate's degree in OT, active OTA licensure in Wisconsin, and commitment to continuing education.

Qualifications

• Associate's degree in occupational therapy.
• MUST have active OTA licensure in state(s) of practice
• Ongoing continuing education and professional development to maintain current licensure and certification

Responsibilities

• Screen residents to determine best OT interventions.
• Perform treatment plans and follow goals per plan of care.
• Communicate findings to residents, families, and team.
• Establish resident therapy schedules with the team.
• Deliver therapy per plan and document/bill services.
• Perform other duties to effectively treat patients.
• Comply with CMS documentation standards daily.

Education

Associate's degree in occupational therapy

Active OTA licensure in state(s) of practice

Continuing education to maintain licensure and certification

Job Position:

Certified Occupational Therapy Assistant for an In‑house Skilled Nursing Facility

Job Type:

Part Time or PRN positions available

Job Purpose:

To plan, organize, develop, and direct Occupational Therapy Services in facility in accordance with current applicable federal, state, and local standards, guidelines, and regulations, and as may be directed by Administrator, to ensure that highest degree of quality resident care can be maintained at all times.

Job Functions

• Conduct regular screening of residents to figure out the best intervention or treatment for each individual patient based off scope of practice
• Perform treatment plan to residents; follow short and long term goals and methods to achieve goals based off of the established plan of care by supervising therapist
• Interpret and communicate evaluation findings, treatment plans, and recommendations to residents, families/caregivers, and interdisciplinary team members
• Establish individual resident therapy schedules in cooperation with other team members
• Conduct therapy according to established treatment plan and ensure proper documentation and billing of occupational therapy services
• Perform all other duties as assigned and as required to effectively treat patients
• Comply with all CMS documentation standards of practice daily with point of care documentations as indicated

Education & Qualifications
