Job summary

Saber Healthcare Group is seeking a full-time OT/OTA to help return patients to optimal function in a skilled nursing facility. You will guide patients toward therapy goals and discharge readiness under supervision.

Responsibilities include educating patients, creating independent exercise plans, and adjusting activities to maximize safety and progress. Requires an associate degree, OTA license, and current state credentials.

Qualifications

• Associate degree from an accredited college.
• Current and valid license issued by the state of employment.
• Current OTA license issued by the state of employment.
• Basic computer skills and ability to navigate service delivery software and electronic health record software.

• Restore patient to optimal physical function and support therapy goals in a SNF setting.
• Instruct patients on maintaining function after discharge via education and independent exercise.
• Provide activity modification guidance under supervision of OT and DOR.
• Collaborate with therapy team to ensure patient progress and safety.

Position Summary:

The primary function of your job return patient to optimal physical function and facilitate the achievement of patient, physician, and therapy goals within a SNF setting. Instruct the patient in maintaining achieved level of function after discharge through education, independent exercise program, and appropriate activity modification, under the supervision of the OT and DOR.

Education/Qualifications:

• Employee must hold an Associate’s Degree from an accredited college.

• Must hold a current and valid license granted from the state of employment.

• Current and valid OTA license granted from state of employment.

• Must possess basic computer skills and ability to navigate through various software programs, including service delivery software and the electronic health record software.

Shift & Wage:

• Full-Time Opportunity Available

• Shift Needed: Days

• Wage: $28.00 - $34.00 per hour
