Saber Healthcare Group seeks a dedicated Occupational Therapy Assistant to help residents regain function in a skilled nursing facility. You will assist with therapy plans, guide patients through independent exercises, and modify activities to fit each resident’s needs under supervision.

In this role, you will work closely with the OT and Rehab team to achieve goals, ensuring safe discharge planning and ongoing support. A strong clinical foundation and computer skills are essential.

Qualifications

• Associate’s Degree from an accredited college is required.
• Current and valid license from the state of employment.
• Current and valid OTA license from the state of employment.
• Basic computer skills and ability to navigate software including EHR.

Responsabilités

• Instruct patients in maintenance after discharge with independent exercise and activity modification under supervision.

Position Summary:

The primary function of your job return patient to optimal physical function and facilitate the achievement of patient, physician, and therapy goals within a SNF setting. Instruct the patient in maintaining achieved level of function after discharge through education, independent exercise program, and appropriate activity modification, under the supervision of the OT and DOR.

Education/Qualifications:

• Employee must hold an Associate’s Degree from an accredited college.

• Must hold a current and valid license granted from the state of employment.

• Current and valid OTA license granted from state of employment.

• Must possess basic computer skills and ability to navigate through various software programs, including service delivery software and the electronic health record software.
