Saber Health Group is seeking a dedicated therapy professional to help residents regain physical function in a skilled nursing facility. You will guide patients toward therapy goals, educate on post-discharge exercise, and work under the supervision of the OT and DOR.

The position emphasizes a strong support environment, continuing education opportunities, and a focus on patient-centered care in a team-driven setting.

Qualifications

• Employee must hold an Associate’s Degree from an accredited college.
• Must hold a current and valid license granted from the state of employment.
• Current and valid OTA license granted from state of employment.
• Must possess basic computer skills and ability to navigate through various software programs, including EHR software.

Responsabilités

• Instruct the patient in maintaining achieved level of function after discharge through education and an independent exercise program.
• Provide activity modification guidance under supervision of the OT and DOR.

Position Summary:

The primary function of your job return patient to optimal physical function and facilitate the achievement of patient, physician, and therapy goals within a SNF setting. Instruct the patient in maintaining achieved level of function after discharge through education, independent exercise program, and appropriate activity modification, under the supervision of the OT and DOR.

Education/Qualifications:

• Employee must hold an Associate’s Degree from an accredited college.
• Must hold a current and valid license granted from the state of employment.
• Current and valid OTA license granted from state of employment.
• Must possess basic computer skills and ability to navigate through various software programs, including service delivery software and the electronic health record software.

Shift & Wage:

• Full-Time Opportunity Available
• Shifts Needed: Monday - Friday
• Wage: $28.00 - $35.00 per hour
