Job summary

Saber Health Group is seeking an OTA-licensed clinician to help residents in a skilled nursing facility regain function and independence. You will educate patients, guide independent exercise, and support therapy goals under supervision of the OT and DOR.

Qualifications

• Associate degree from an accredited college is required.
• Current and valid state license for OTA practice is required.
• Basic computer skills and ability to use EHR and service delivery software.

Responsibilities

• Educate patients to maintain function after discharge with home exercise and activity modification.
• Collaborate with OT and Director of Rehabilitation to achieve patient goals.

Position Summary:

The primary function of your job return patient to optimal physical function and facilitate the achievement of patient, physician, and therapy goals within a SNF setting. Instruct the patient in maintaining achieved level of function after discharge through education, independent exercise program, and appropriate activity modification, under the supervision of the OT and DOR.

Education/Qualifications:

• Employee must hold an Associate’s Degree from an accredited college.
• Must hold a current and valid license granted from the state of employment.
• Current and valid OTA license granted from state of employment.
• Must possess basic computer skills and ability to navigate through various software programs, including service delivery software and the electronic health record software.
