Delta Health and Rehabilitation in Delta, CO is seeking a full-time Occupational Therapy Assistant to join our in-house therapy team. You will assess patients with physical functioning disorders, develop and implement comprehensive care plans, and deliver rehabilitative interventions with a focus on wound care and mobility.

Responsibilities

• Screen and evaluate patients with physical functioning disorders and/or wound care; develop care plans per standards.
• Deliver rehabilitative, skilled, and medically necessary interventions per regulatory and clinical practice requirements.
• Stand and walk regularly; assist with lifting, transferring and repositioning as needed.
• Administer procedures as part of the rehabilitation plan: manual techniques, ambulation, therapeutic exercises, modalities, devices, wound care.
• Provide consultation to patients, families, caregivers and other providers regarding physical disorders.
• Generate discharge summaries ensuring regulatory and clinical compliance.

OCCUPATIONAL THERAPY ASSISTANT - FULL TIME

DELTA HEALTH AND REHABILITATION - DELTA, CO

We are excited to announce our growth in Colorado and are currently seeking passionate Occupational Therapy Assistants to join our in-house crew at Delta Health and Rehabilitation. As a therapy professional on our team, you will have the freedom to create and implement programs to best meet the needs of your patients. Our therapy team expansion will be a sharp focus, and you will receive strong orientation, training and on-going support as we grow!

Key Responsibilities

• Conduct effective screening and evaluation of patients with physical functioning disorders and/or wound care, developing appropriate care plans adhering to regulatory and clinical standards.
• Deliver rehabilitative, skilled, and medically necessary treatment interventions to patients with physical functioning disorders and/or wound care, in accordance with regulatory and clinical practice requirements.
• The role involves regular standing and walking throughout the facility and occasional lifting, transferring, and repositioning of patients and equipment. Physical demands vary throughout the day based on patient care needs.
• Administer various procedures as part of the rehabilitation plan, including manual techniques, ambulation, therapeutic exercises, modalities, use of supportive and assistive devices, and wound care.
• Provide consultation and counseling to patients, families, caregivers, and other service providers regarding physical disorders.
• Generate comprehensive discharge summaries of services provided, ensuring compliance with regulatory and clinical requirements.

Qualifications

Occupational Therapy Assistant license is required.
