Backed by top VCs, Seen Health is a culturally-focused, technology-enabled healthcare organization that integrates comprehensive medical care and social support with a high-touch, interdisciplinary approach.

Our mission is to empower seniors to age-in-place with dignity and provide their families peace of mind. We are building upon a proven Home and community based services model to create a culturally-competent and scalable PACE program. We are also building a comprehensive operating system focused on data and workflows that span across systems, processes, people, and care contexts. We want to empower our clinicians and staff with tools that deliver relevant data at the time and site of care and enable them to deliver exceptional care to our participants, which improve clinical outcomes, participant & provider satisfaction, and ultimately our strength as an organization.

We are a mission-driven, multidisciplinary team with deep healthcare, technology, and operations expertise, each inspired by our own personal stories of caring for seniors in our lives. Our name, Seen Health, was chosen to reflect our commitment to provide the highest standard of care to underserved older adults while respecting and incorporating their individual beliefs, heritage, and values, so that they can truly be seen.

About The Role

The Occupational Therapy Assistant will work closely with the Occupational Therapists and the interdisciplinary team to help our seniors maintain and improve their independence, safety, and ability to perform daily activities. They will provide therapeutic interventions, monitor and document participant progress, educate participants and caregivers on adaptive strategies and equipment, support care planning, and collaborate across the care.

Responsibilities

  • Reinforce therapy recommendations and support participants in developing, maintaining, and restoring functional abilities that promote aging in place and community living.

  • Observe participant progress and response to treatment, documenting findings and communicating relevant clinical information to the Occupational Therapist and Interdisciplinary Team (IDT).

  • Assist with functional assessments, care planning activities, and identification of participant needs, risks, and opportunities for intervention.

  • Educate participants, caregivers, and care team members on therapeutic techniques, adaptive strategies, home safety recommendations, and the appropriate use of adaptive equipment and assistive technology.

  • Complete timely and accurate documentation within the electronic medical record (EMR) in accordance with organizational, regulatory, and professional standards.

  • Collaborate with the Interdisciplinary Team (IDT) to coordinate participant care, contribute to care plan development, and support positive participant outcomes.

  • Participate in team meetings, quality improvement initiatives, and other program activities that advance Seen Health's mission and goals.

  • Utilize technology and digital tools to support care coordination, documentation, participant outcomes, and operational excellence.

  • Assist with ordering, tracking, and maintaining therapy equipment and treatment areas, respond appropriately to participant incidents or emergencies, and perform other duties as assigned.

Qualifications

  • Associate's degree from an accredited Occupational Therapy Assistant (OTA) program.

  • Active, unrestricted Certified Occupational Therapy Assistant (COTA) license in California.

  • CPR and First Aid certification, or ability to obtain within 60 days of hire.

  • Experience working with older adults and individuals with complex medical, cognitive, and functional needs.

  • Knowledge of occupational therapy interventions, functional assessments, adaptive equipment, assistive technology, and durable medical equipment (DME).

  • Proficiency with technology, including electronic health records (EHRs), care management platforms, communication tools (e.g., Slack, Microsoft Teams), and digital documentation systems.

  • Strong verbal and written communication skills, with the ability to effectively educate participants, caregivers, and interdisciplinary team members.

  • Ability to collaborate effectively within an interdisciplinary care team.

  • Excellent organizational skills, attention to detail, and ability to manage competing priorities.

  • Bilingual in English and Mandarin or Cantonese.

Preferred Skills

  • Familiarity with AI-powered tools and technology that support clinical workflows, documentation, and operational efficiency.

  • Demonstrated ability to thrive in a fast-paced environment, effectively balancing changing participant needs, urgent clinical updates, and cross-functional coordination.
