Anova Care is seeking a licensed Occupational Therapist to provide home health services to patients in the South Denver area. The OT will be responsible for patient assessment, treatment, and documentation in a home-based setting.

Responsibilities include evaluations, treatment planning, and collaboration with families and interdisciplinary teams, ensuring compliance with home health regulations and high-quality care delivery.

Qualifications

• Master’s or Doctorate in Occupational Therapy required.
• Current state licensure as an Occupational Therapist.
• 1–3 years clinical experience in home health, outpatient, or rehabilitation settings preferred.

• Monitor and document patient progress and outcomes in the EMR.
• Communicate effectively with patients, families, and interdisciplinary teams.

• Participate in quality improvement initiatives and care coordination meetings.
• Collaborate with clinical, administrative, and operations teams to support patient care and service delivery.
• Perform other clinical or administrative duties as assigned.

• Knowledge of home health regulations and documentation requirements
• Proficiency with EMR systems and Microsoft Office Suite
• Excellent written and verbal communication and interpersonal skills
• Ability to work independently and manage a patient caseload
• Availability during business hours with occasional after-hours/weekend coverage

Indirect Reports

• Home Health Aides (HHAs) when supporting therapy plans, if applicable
• Certified Occupational Therapist Assistant

Schedule

• Monday–Friday, 8-hour shifts; weekends as needed

Pay

$95,000.00 - $105,000.00 per year
