Job summary

Vertis Therapy is seeking a passionate Occupational Therapist for our Skilled Nursing Facility in Middletown, IN. You will evaluate and treat residents to improve independence and quality of life, and develop individualized treatment plans.

You will monitor progress, document thoroughly, and provide supervision to Certified Occupational Therapy Assistants as appropriate. Join a collaborative interdisciplinary team.

Qualifications

• Degree in Occupational Therapy from an accredited program.
• Current or eligible OT license in the state.
• Strong clinical assessment and ability to develop treatment plans.

Responsibilities

• Evaluate residents' functional abilities and establish care plans.
• Provide skilled occupational therapy services to residents.
• Supervise and guide Certified Occupational Therapy Assistants (COTAs).
• Complete timely documentation and progress notes.
• Lead or participate in team development and wellness activities.
