Job summary

Vertis Therapy is seeking a passionate PRN Occupational Therapy Assistant for our Skilled Nursing Facility in Middletown, IN. You will provide skilled OT services under the supervision of an OT, helping residents improve independence, safety, and quality of life.

The OTA will implement treatment plans, document responses, and collaborate with the interdisciplinary team to monitor progress and adjust care as needed. A flexible schedule with consistent hours each week is offered.

Qualifications

  • Associate degree from an accredited OTA program.
  • Valid Occupational Therapy Assistant state license or license eligibility.
  • Ability to lift and assist residents; stand for extended periods.
  • Computer proficiency to document in an electronic medical record platform.

Responsibilities

  • Provide skilled occupational therapy interventions under the direction and supervision of the OT. Follow individualized treatment plans to improve independence, safety, and functional abilities.
  • Complete timely and accurate documentation of treatments provided and resident progress toward goals.
  • Communicate regularly with the OT and interdisciplinary team regarding progress and changes in function.
  • Participate in team development meetings, trainings, and continuing education to support quality patient care.
  • Lead wellness programming to promote functional independence and safety in the community.

Vertis Therapy is looking for a passionate PRN Occupational Therapy Assistant for consistent hours at our Skilled Nursing Facility in Middletown, IN. This is a great opportunity for a therapist looking for a flexible schedule and consistent hours each week. In this role, you will provide skilled occupational therapy services under the supervision of an Occupational Therapist, helping residents improve their independence, safety, functional abilities, and overall quality of life.

The Certified Occupational Therapy Assistant will carry out individualized treatment plans developed by the Occupational Therapist, provide skilled occupational therapy interventions, monitor and document resident responses to treatment, and collaborate with the interdisciplinary team. The COTA will communicate with the Occupational Therapist regarding resident progress, changes in functional status, and any concerns related to the established plan of care.

Responsibilities:

Treatment: Provide skilled occupational therapy interventions under the direction and supervision of an Occupational Therapist. Follow individualized treatment plans developed by the OT to help residents improve independence, safety, functional abilities, and quality of life.

Documentation: Complete timely and accurate documentation of treatments provided, resident response to therapy, and progress toward established goals.

Collaboration: Communicate regularly with the Occupational Therapist and interdisciplinary team regarding resident progress, response to treatment, and any changes in functional status or needs.

Professional Development: Participate in team development meetings, training opportunities, and continuing education activities to maintain clinical knowledge and support quality patient care.

Wellness Programming: Participate in and regularly lead community-wide wellness programs and events that promote functional independence, safety, mobility, and overall well-being.

Qualifications:

Education: Associate degree from an accredited Occupational Therapy Assistant program.

Licensure: Valid Occupational Therapy Assistant state license or license eligibility.

Physical Requirements: Ability to lift and assist residents as needed. Must be able to stand for extended periods and perform physical tasks related to therapy.

Technology: Computer proficiency to document in an electronic medical record platform.
