Job summary

Vertis Therapy is seeking a PRN Occupational Therapy Assistant for our Skilled Nursing Facility in Middletown, IN. The role offers flexible hours and consistent weekly workload, providing skilled OT services under the supervision of an Occupational Therapist to improve residents’ independence and quality of life.

The OTA will follow OT plans, document progress, and collaborate with the interdisciplinary team.

Qualifications

• Associate degree in OTA required with state licensure or eligibility.
• Ability to perform skilled OT interventions under supervision.
• Competence in documenting resident progress and outcomes.

Responsibilities

• Provide skilled occupational therapy interventions under the direction and supervision of an Occupational Therapist.
• Complete timely and accurate documentation of treatments, resident responses, and progress toward goals.
• Communicate with the OT and interdisciplinary team regarding resident progress and changes in status.
• Participate in team development meetings and continuing education activities.
• Lead community-wide wellness programs to promote independence, safety, mobility and overall well-being.
