Job summary

Cooper River Post Acute in Mount Pleasant, SC is seeking a compassionate Occupational Therapist (PRN) to join our skilled nursing and rehabilitation team. You will evaluate residents, create individualized plans, and work closely with families and staff to restore independence.

Ideal candidates hold SC OT licensure, strong communication, and documentation skills, with SNF/rehab experience preferred. We offer a supportive team, ongoing education, and a rewarding clinical environment.

Qualifications

• Graduate of an accredited Occupational Therapy program.
• Current OT license in the state of South Carolina.
• Strong communication and interpersonal skills.
• Excellent organizational and documentation skills.
• A compassionate, patient-centered approach.
• Ability to work effectively as part of a team.
• Previous skilled nursing/rehab experience is preferred, but we're happy to consider the right candidate!

Responsibilities

• Evaluate residents and develop individualized treatment plans.
• Provide skilled OT services based on residents' needs and goals.
• Help residents improve strength, coordination, mobility, and independence in daily activities.
• Provide training and education to residents, families, and caregivers.
• Monitor and document residents' progress.
• Collaborate with interdisciplinary team to achieve best outcomes.
• Maintain accurate and timely clinical documentation.
• Participate in care planning and communicate progress recommendations.
• Provide compassionate, resident-centered care.
• Supervise COTA, aides, and students.
