Job summary

Cooper River Post Acute in Mount Pleasant, SC is seeking a full-time Occupational Therapist. The OT will assess residents, create individualized treatment plans, and provide skilled therapy to improve daily functioning and independence.

You will work closely with families and the interdisciplinary team to deliver high-quality care. The ideal candidate holds an OT license in South Carolina, graduated from an accredited OT program, and brings strong communication and documentation skills.

Qualifications

• Graduate of an accredited Occupational Therapy program.
• Current OT license in the state of South Carolina.
• Excellent communication and documentation skills.
• Experience in skilled nursing/rehab preferred.
• Ability to work within an interdisciplinary team.

Responsibilities

• Evaluate residents and develop individualized treatment plans.
• Provide skilled OT services based on residents' needs and goals.
• Improve mobility, strength, balance, and independence.
• Educate residents, families, and caregivers.
• Document progress and maintain records.
• Collaborate with interdisciplinary team to achieve outcomes.
• Supervise COTA and aides as needed.
• Complete timely, accurate documentation.
• Report equipment needs and ensure safety.
