Job summary

Cooper River Post Acute in Mount Pleasant, SC, a 176-bed skilled nursing and rehabilitation facility, is seeking a PRN Occupational Therapist to join our team. You’ll evaluate residents, design individualized treatment plans, and provide skilled OT services to help residents regain independence and improve daily living skills.

You’ll collaborate with physicians, nursing staff, families, and an interdisciplinary team, maintain accurate documentation, and participate in care planning.

Qualifications

• Graduate of an accredited Occupational Therapy program.
• Current SC OT license.
• Strong communication and interpersonal skills.
• Excellent organizational and documentation skills.
• Compassionate, patient-centered approach.
• Ability to work effectively as part of a team.
• SNF/Rehab experience preferred.

Responsibilities

• Evaluate residents and develop individualized treatment plans.
• Provide skilled OT services based on residents’ needs and goals.
• Help residents improve strength, coordination, mobility, and independence with daily activities.
• Provide training and education to residents, families, and caregivers.
• Monitor and document residents’ progress.
• Collaborate with the interdisciplinary team to achieve the best outcomes.
• Maintain accurate and timely clinical documentation.
• Participate in care planning and communicate recommendations regarding resident progress.
