Job summary

Oakwood Gardens Care Center in Fresno, CA is seeking a licensed Occupational Therapist to provide skilled OT services in a SNF setting, focused on resident-centered outcomes.

You will evaluate residents, develop tailored treatment plans, supervise COTAs, document progress, and participate in discharge planning with the care team.

A California OT license is required; Casamba or PointClickCare EMR experience is a plus; growth opportunities and comprehensive benefits offered.

Qualifications

• California OT license required.
• EMR experience with Casamba or PointClickCare is a plus.
• SNF or post-acute rehab experience preferred but not required.

Responsibilities

• Evaluate residents to determine functional abilities and therapy needs.
• Develop and implement individualized treatment plans.
• Provide direct therapy services per physician orders.
• Supervise COTAs and collaborate with the interdisciplinary care team.
• Document treatment progress and outcomes per regulatory standards.
• Participate in discharge planning and caregiver education.
• Contribute to in-service training and quality improvement initiatives.
