Job summary

Enhance Rehabilitation is seeking an Occupational Therapist to join our team in a SNF setting, evaluating and treating patients following individualized plans of care. You will collaborate with facility staff to ensure high-quality patient care, provide education to patients and families, and complete timely documentation.

You will also supervise Occupational Therapy Assistants to support therapy goals and maintain quality patient outcomes.

Responsibilities

• Evaluate and treat patients in a skilled nursing facility following an individualized plan of care.
• Collaborate with facility staff to ensure the highest standard of patient care.
• Develop and implement plans of care and set measurable, functional goals.
• Complete timely and accurate documentation including evaluations, re-evaluations, discharges and daily progress notes.
• Supervise Occupational Therapy Assistants to ensure quality patient outcomes.
• Educate patients and families about injury, progress, and expected outcomes.
