Job summary

Enhance Rehabilitation is seeking a qualified Occupational Therapist to join our dynamic SNF team. You will evaluate and treat patients following individualized care plans, collaborating with facility staff to ensure excellent patient outcomes.

The role includes developing and adjusting plans of care, setting functional goals, and maintaining thorough documentation. Supervising Occupational Therapy Assistants and communicating progress to patients and families are essential parts of the position.

Qualifications

• OT degree required and current state licensure/eligensure as applicable.
• Experience evaluating and treating patients in SNF settings preferred.
• Strong documentation and communication skills required.

Responsibilities

• Evaluate and treat patients in SNF following an individualized plan of care.
• Develop plans of care and set measurable, functional goals.
• Re-evaluate patient progress and revise plans of care as needed.
• Complete timely documentation including evaluations, re-evaluations, discharges, and daily progress notes.
• Supervise Occupational Therapy Assistants to ensure quality of care.
