Job summary

Enhance Rehabilitation is seeking qualified Occupational Therapist candidates to join our dynamic team in a SNF setting, evaluating and treating patients following an individualized plan of care.

You will evaluate new patients, develop plans of care with measurable goals, treat patients, document progress, and supervise Occupational Therapy Assistants while collaborating with facility staff to ensure high standards of patient care.

Responsibilities

• Providing excellent care for our patients
• Providing excellent customer service to our patients and family members
• Collaborate with facility staff to ensure excellent care for you patients
• Evaluating new patients, developing plans of care and setting measurable, functional goals
• Treating patients, answering their concerns and educating them on their injury, progress and probable outcomes
• Re-evaluating patient progress, revising plans of care and resetting goals
• Complete timely and accurate documentation including; evaluations, re-evaluations discharges and daily progress notes
• Supervising Occupational Therapy Assistants
