Job summary

Enhance Rehabilitation is seeking a qualified Occupational Therapist to join our team in a SNF setting. You will evaluate and treat patients following an individualized plan of care, collaborating with facility staff to ensure excellent patient outcomes.

You will supervise Occupational Therapy Assistants and maintain comprehensive documentation, including evaluations, re-evaluations, discharges and daily progress notes, while educating patients and families on injury and prognosis.

Responsibilities

• Evaluate new patients and develop plans of care
• Treat patients and educate them on injury, progress and outcomes
• Re-evaluate patient progress and revise plans of care
• Complete timely documentation of evaluations, progress notes and discharges
• Supervise Occupational Therapy Assistants
