Job summary

Enhance Rehabilitation in Illinois is seeking qualified Occupational Therapist candidates to join our dynamic SNF care team. You will evaluate and treat patients in a skilled nursing facility, following an individualized plan of care with measurable goals tailored to each patient’s needs.

You will collaborate with facility staff to ensure excellent patient care, educate patients and families on progress and probable outcomes, and supervise Occupational Therapy Assistants.

Responsibilities

• Evaluate and treat patients in a SNF setting.
• Collaborate with facility staff to ensure excellent patient care.
• Develop plans of care and set measurable functional goals.
• Treat patients, address concerns, and educate them on injury, progress and probable outcomes.
• Re-evaluate patient progress, revise plans of care and reset goals.
• Complete timely and accurate documentation including evaluations, re-evaluations, discharges and daily progress notes.
• Supervise Occupational Therapy Assistants.
