Sweetwater Care is seeking an Occupational Therapist to provide rehabilitative therapy in a skilled nursing setting. The role requires a current Ohio license, and 1+ years of SNF experience. You will screen patients, plan care, implement therapy, and document progress for regulatory compliance.

You may supervise PTAs and rehabilitation aides, contribute to care plans, and participate in team meetings to support patient outcomes and quality of life.

Formación

• Bachelor's degree or higher in Occupational Therapy.
• Active Ohio state license.
• 1+ years of experience in a skilled nursing facility.

Responsabilidades

• Screen/evaluate patients and develop plan of care for cognitive and ADL disorders.
• Use standardized assessments and evidence-based practice.
• Provide rehabilitative treatment per regulatory standards.
• Document clinical progress and discharge summaries.
