Job summary

You will document progress, participate in mandatory trainings, and provide discharge summaries while ensuring compliance with regulatory requirements and professional standards.

Qualifications

• Active State Licensure required.

Responsibilities

• Consult with patients to learn about their physical condition and symptoms.
• Review medical history and referrals from doctors or surgeons.
• Diagnose movement dysfunction by observing patients as they move.
• Develop individualized treatment plans.
• Coach patient and teach therapeutic massage to ease pain and promote healing.
• Maintain patient records and track goals.
• Use standardized assessments and evidence-based practice to support interventions.
• Assess progress and adjust treatments in the clinical record.
• Evaluate treatment outcomes and modify services accordingly.
• Provide consultation to patients, families, caregivers and other providers.
• Communicate progress and plans with supervisor and health team.
• Provide comprehensive discharge summaries as required.
• Document all regulatory information in the electronic system.
• Participate in trainings, in-services and conferences.
• Able to supervise PT assistants, aides and students.
• Adhere to policy, procedures and code of conduct.
• Maintain active licensure.
