Job summary

Stellar Health Group in Georgia, VT is seeking an Occupational Therapist (OT) to join our team at a Skilled Nursing Facility. You will evaluate residents, develop individualized treatment plans, and help patients regain independence through tailored activities.

Responsibilities include monitoring progress, documenting reports, rehabilitating residents with disabilities, and coordinating with healthcare teams. Current Vermont OTR or COTA license is required; long-term care experience preferred.

Qualifications

• Graduate of an accredited Occupational Therapy Program.
• Current Vermont OTR or COTA License.
• Experience in long-term care settings, preferred.

Responsibilities

• Evaluating residents and developing individualized treatment plans to meet their unique needs.
• Choosing activities that promote independence and daily living skills.
• Monitoring resident progress and preparing detailed reports.
• Rehabilitating residents with disabilities due to injury, illness, or other conditions.
• Maintaining a safe and clean working environment.
• Collaborating with healthcare teams to ensure seamless care coordination.
• Supervising COTAs as required.
