Job summary

Stellar Health Group in Georgia, VT is seeking an Occupational Therapist to join our Skilled Nursing Facility team. The OT will evaluate residents, develop plans, and support daily living skills, in partnership with the care team.

Qualifications

• Graduate of an accredited Occupational Therapy Program.
• Licensed OTR or COTA in Vermont.
• Experience in long-term care settings preferred.

Responsibilities

• Evaluate residents and develop individualized treatment plans.
• Choose activities that promote independence and daily living skills.
• Monitor resident progress and prepare detailed reports.
• Rehabilitate residents with disabilities due to injury, illness, or other conditions.
• Maintain a safe and clean working environment.
• Collaborate with healthcare teams to ensure seamless care coordination.
• Supervise COTAs as required.
