Qualifications

• Requires active OT license in the state of practice.
• Experience in occupational therapy preferred.
• Ability to communicate effectively with patients and the care team.

Responsibilities

• Coordinate and deliver occupational therapy services for all patient ages.
• Develop and implement individualized treatment plans based on assessments.
• Gather data from records, interviews, observations and team members.
• Provide direct therapy using various functional settings and strategies.
• Coordinate therapy goals with rehabilitation and nursing care plans.
• Prepare documentation such as evaluation results and progress notes.
• Communicate patient status and needs with family, physicians and team.
• Collaborate with interdisciplinary team and participate in program development.
