Job summary

Vista Ridge Care and Rehabilitation is seeking an Occupational Therapist to provide rehabilitative therapy in a skilled nursing environment. The role involves evaluating patients, planning and delivering interventions, and documenting outcomes to support improved function and independence.

Reports to the Director of Rehabilitation Services; state licensure and SNF experience are required. We offer competitive pay ($45–$55 per hour), a full-time/part-time/PRN schedule, and benefits including

Qualifications

• Must have a Bachelor's degree or higher in Occupational Therapy or related field.
• Active Kansas state license is required.
• 1+ years of experience in a skilled nursing facility environment.

Responsibilities

• Screen and evaluate patients with cognitive and/or ADL impairments and develop care plans.
• Apply standardized assessments and evidence-based interventions.
• Provide rehabilitative therapy and documentation per regulatory standards.
• Deliver ADL retraining, therapeutic activities, exercises, and modalities as prescribed.
• Monitor patient progress and adjust treatments in medical records.
• Communicate progress and plans with supervisors and health team members.
• Prepare discharge summaries and maintain comprehensive patient records.
