Job summary

Enhance Rehabilitation is seeking a qualified Occupational Therapist to join our team in a SNF setting. You will assess and treat patients, develop plans of care, and work with facility staff to deliver high-quality care.

You will also supervise Occupational Therapy Assistants, document progress, and collaborate with families to educate them on progress and outcomes. A sign-on bonus is available for this position.

Responsibilities

• Evaluate and treat patients in a SNF setting following an individualized plan of care
• Provide high-quality patient care and customer service to patients and families
• Collaborate with facility staff to ensure excellent patient outcomes
• Evaluate new patients, develop plans of care and set measurable goals
• Treat patients, address concerns and educate on injury, progress and outcomes
• Re-evaluate patient progress, revise plans of care and reset goals
• Complete timely and accurate documentation including evaluations, re-evaluations, discharges and daily progress notes
• Supervise Occupational Therapy Assistants
