Job summary

Ultimate Therapy is hiring an Occupational Therapist (OT) for Autumn Lake at Norwalk, CT. You will evaluate residents, develop plans of care, and address daily living activities, upper extremity function, balance, cognition, and safety.

This full-time role collaborates with PT, SLP, nursing and the broader team to coordinate resident care. The position emphasizes compassionate care, ongoing documentation, and opportunities for professional development in a team-based rehabilitation setting.

Qualifications

• Active Connecticut Occupational Therapist license required.
• Graduate of an accredited Occupational Therapy program.
• Strong assessment, communication, and organizational skills.
• New graduates are welcome to apply.
• Skilled nursing experience preferred, but not required.

Responsibilities

• Evaluate residents and develop personalized occupational therapy plans of care.
• Provide interventions addressing activities of daily living, upper extremity function, coordination, balance, cognition, and safety.
• Help residents develop strategies for greater independence with everyday routines.
• Assess progress and adjust treatment approaches according to resident needs.
• Recommend adaptive equipment and provide resident and caregiver education.
• Maintain thorough and timely clinical documentation.
• Partner with PT, SLP, nursing, and the interdisciplinary team to coordinate resident care.
