Job summary

Ultimate Therapy is seeking an Occupational Therapist (OT) to join the rehabilitation team at Autumn Lake at Salem County in Salem, NJ. The role focuses on helping residents regain independence and improve daily function within a skilled nursing setting.

You will evaluate residents, develop individualized OT plans, and collaborate with PT, SLP, nursing, and other team members. A NJ OT license and an accredited OT program graduate are required; new graduates are welcome.

Qualifications

• Strong clinical, communication, and organizational skills.
• Active New Jersey Occupational Therapist license required.
• Graduate of an accredited Occupational Therapy program.

Responsibilities

• Evaluate residents and develop individualized occupational therapy treatment plans.
• Provide skilled interventions focused on ADLs, strength, coordination, balance, cognition, and functional mobility.
• Help residents improve independence with daily tasks and routines.
• Recommend adaptive equipment and educate residents and caregivers on proper use.
• Monitor progress and adjust treatment plans based on resident needs.
• Complete accurate and timely clinical documentation.
• Collaborate with PT, SLP, nursing, and other care team members.
