Job summary

Ultimate Therapy Services is seeking a Certified Occupational Therapy Assistant (COTA) to join the rehabilitation team at Autumn Lake at Norwalk in Norwalk, CT. The role supports residents in completing daily activities, with supervision from a licensed OT.

Responsibilities include providing treatments, assisting with ADLs, following treatment plans, documenting progress, and collaborating with therapy, nursing, and care teams.

Qualifications

• Active Connecticut COTA license required.
• Strong communication and resident-care skills.
• New graduates are encouraged to apply.

Responsibilities

• Provide occupational therapy treatments under the supervision of a licensed OT.
• Assist residents with activities of daily living, strengthening, coordination, balance, and functional tasks.
• Follow individualized treatment plans and support resident-specific rehabilitation goals.
• Observe progress and communicate treatment updates to the supervising OT.
• Reinforce adaptive techniques and safety strategies with residents and caregivers.
• Maintain accurate and timely clinical documentation.
• Work collaboratively with therapy, nursing, and other care-team members.
