Job summary

Westwood Post Acute in San Jose, CA is hiring an Occupational Therapist (OT) to deliver and direct all aspects of patient care, including evaluation, treatment planning, and documentation. Newly licensed OT graduates are welcome; SNF/Long-Term Care experience is a plus.

Qualifications

• Newly licensed OT/new OT graduates are welcome.
• Experience in Skilled Nursing or Long-Term Care is a plus.
• California license to practice Occupational Therapy is required.
• BLS Certification is required.

Responsibilities

• Conduct comprehensive assessments to identify patients' strengths, limitations, and goals.
• Develop individualized treatment plans based on the patient’s needs and goals.
• Provide therapeutic interventions to improve patients' physical, cognitive, and emotional well-being.
• Collaborate with other healthcare professionals to ensure coordinated care.
• Educate patients and their families on adaptive techniques for independence.
• Document progress and update treatment plans as needed.
